Day-Break Adult Day Center — a great option for many seniors.

Staying home is not the only option for patients who are homebound. As a medical model adult day program, Day-Break has the resources and structure necessary to accommodate the needs of many seniors—from assisting with ADLs to insulin administration and Alzheimer’s support. At Day-Break we help our clients understand what is expected of them in terms of medication, doctor’s visits, and following a general healthy lifestyle. This is especially important for patients needing extra assistance for chronic conditions, or recuperating from hospitalization.

The structure and support of an adult day center can often help reduce the chances of readmission. At Day-Break we monitor key health metrics, including changes in appetite and energy, and can report and respond to changes before they escalate. Our social activities, assistance with hygiene and ADL’s enhance our client’s willingness to follow a healthy regimen designed for recovery or improved day-to-day health management.

The Day-Break staff will also assist families and professionals with coordination and referrals to other services such as housing, home delivered meals, transportation, and more. With us, you can be sure that the people you refer are being cared for safely and reliably, and at a lower cost than most in-home providers.

Administer medications, monitor vitals and deliver follow-up wound care to maintain your health.
Health Management and Monitoring
Manage and monitor Alzheimer's, diabetes, hypertension, muscle impairments, cognition and more. We also provide guidance to help you understand any health concerns you might have, as well as maintain your health.
Coordination with Personal Physicians
Regularly communicate with your personal physicians to help you stay on top of your health.
Everyday Personal Care
If you need support with personal care, we’re here to help. Some of our members even shower here, so they return home well-cared for, refreshed and ready to enjoy their evening.

Day-Break participates in the Connecticut Adult and Child Food Program (CACFP), which has a rigorous set of guidelines for balanced diet and nutrition. We provide breakfast and lunch, as well as a snack each day, and we also customize meals for specialty diets.
